Foot Drop

Foot drop, also known as drop foot, is dependent upon the extent of muscle weakness or paralysis and refers to the dropping of the forefoot and/or the inability to lift the foot from the ankle or to lift the toes.  Either a temporary or permanent condition, this symptom can be caused by a disease, stroke, amyotrophic lateral sclerosis, multiple sclerosis, muscular dystrophy, cerebral palsy, sciatic nerve damage, fibular nerve damage, spinal cord trauma, toxins, or hereditary spastic paraplegia.

What is Foot Drop?

Characterized by steppage gait, foot drop refers to the condition of experiencing the inability to lift the foot from the ankle and/or to lift the toes. This causes individuals to bend their knees while walking to avoid dragging their feet along the ground. If the toes can’t be lifted on one foot, an affected person may also tiptoe walk on the opposite leg, excessively raising the thigh as one would normally do walking up stairs and steps. Another gait style indicative of foot drop includes a wide outward swing of the legs, especially when turning corners in the opposite direction of the affected foot.

As mentioned above, there can be a variety of root problems that cause foot drop, including disease. Stroke is one culprit, while others include ALS (amyotrophic lateral sclerosis), MS (multiple sclerosis), MD (muscular dystrophy), CP (cerebral palsy), Friedreich’s ataxia, Charcot-Marie Tooth disease and hereditary spastic paraplegia. This condition can also occur as a side effect of hip replacement surgery. And as all-around healthy as yoga practice can be, it is important to note a condition known as ‘yoga foot drop’. Caused by prolonged sitting on the heels, nerve damage or deadening can occur in the nerve that runs under the knee if it is deprived of oxygen for too long while kneeling.

How to Correct Foot Drop Using an Ankle-Foot Orthosis (AFO)

Also known as a foot-drop brace, an ankle-foot orthosis (AFO) is one of the most commonly used orthoses, comprising about 26% of all orthoses used in the USA. Typically constructed of lightweight polypropylene-based plastic formed into an “L” shape, the upright portion runs behind the leg calf, with the lower portion sliding under the foot. Some “L” designs are unbroken providing greater rigidity, while others may feature a jointed ankle to offer other kinds of control. AFOs used for foot drop can also include designs created from other lightweight molded plastics and soft, elastic materials. Another method for correcting foot drop employs the use of a cuff placed around the patient’s ankle, with a topside hook and spring installed under the shoelaces. Because the hook connects to the cuff, it lifts the shoe up as the patient walks. Spring-loaded braces fitted inside the shoes also work to correct foot drop in this similar fashion.

There are six basic AFOs that physicians generally recommend the most for their patients with foot drop to help provide a more normal and comfortable gait. These are comprised of a Short Leg AFO with Fixed Hinge, a Solid Ankle AFO, a Full Leg Posterior Leaf Spring AFO, a Dorsiflexion Assist Short Leg AFO, an Energy Return AFO and a Plantarflexion Stop AFO. Here is some more information about each choice:

Short Leg AFO with Fixed Hinge:
Relatively lightweight, this type of AFO is easy to fit into shoes and offers superior control of the foot. Keeping the foot at a 90-degree angle to the leg, this orthosis can also help those with flat feet. It additionally helps control unwanted inward rotation of the foot, which often occurs in patients recovering from stroke, or in those with Charcot-Marie Tooth disease. This Short Leg AFO works best for individuals under 6 feet tall, and does not allow for plantarflexion or dorsiflexion.

Solid Ankle AFO:
Perfect for foot drop patients who have lost most or all of their dorsiflexion strength, and who also suffer from unstable knees, the Solid Ankle AFO stops plantarflexion and dorsiflexion. It is typically a little bit bulkier, yet it provides a superior amount of control.

Full Leg Posterior Leaf Spring AFO:
As a more traditional AFO, this tried and true style has been around for many years. Although it is not as lightweight as its newer counterparts, it is still a preferred choice for some patients who suffer greater instability of the knees along with foot drop issues.

Dorsiflexion Assist Short Leg AFO:
Incorporating a spring-like hinge to raise the foot, dorsiflexing the ankle when the foot comes off the ground, this style AFO offers a more normal gait pattern. As one of the most recommended AFOs for patients with mild to moderate foot drop challenges, along with flat feet or unstable feet, this style is also referred to as a Dorsiflexion Assist Functional AFO. It is typically only recommended for patients who are less than 6 feet tall, and weigh under 225 pounds.

Energy Return AFO:
A natural flex is built-in to the materials used in this type of AFO to provide supportive dorsiflexion. Often prefabricated using carbon graphite materials, the Energy Return AFO offers great control using very little weight.

Plantarflexion Stop AFO:
Featuring a hinge that allows for normal dorsiflexion, this AFO stops plantarflexion by its ability to stop the foot from pointing downward. While it is somewhat bulkier than other AFOs, it can be the perfect choice for patients with more severe foot drop issues.
