What is a Safety Bed?

Safety beds, hospital beds, and special needs beds all share a common goal of keeping the user safe from falling out of bed or injuring themselves while being in bed. To be considered a safety bed, it must address a few very specific issues:

• Fall protection – some physical barrier should be built into the structure of the bed to minimize the chance that users can fall out of bed. In many cases, this protection has to apply while they are awake and moving around, not just while they are sleeping.
• Protection from entrapment – a reduction of gaps and openings are built into the design of the bed to minimize the chance of the user becoming stuck in the frame or between mattress and frame.
• Caregiver assistance to exit – the safety features should not be able to get accidentally disengaged. Many individuals are unable to safely exit a bed unsupervised as they are a danger to themselves or others.
• One hand operation – openings or doors should be easy to operate with one hand, which allows the free hand to assist the person as needed. At no time should the caregiver have to divert their attention away from the person to operate the unit.

What is Bed Entrapment?

There are seven areas of bed entrapment, places a person can get stuck or injured in and around a bed.

1) Within the bed rail itself. A risk of head entrapment can happen in any open space between the perimeters of the rail. The FDA recommends this space to be less than 4 ¾ inches.

2) Between the rail supports, under the rail, or next to a single rail support. The gap under the rail, between it and the mattress, may allow for dangerous head entrapment. The FDA recommends this space to be less than 4 ¾ inches.

3) Between the mattress and rail. This area is the space between the mattress and the inside surface of the bed rail. If it is too big it can cause a risk of head entrapment. The FDA recommends this space to be less than 4 ¾ inches.

4) Under the ends of the bed rail. A gap between the mattress and the lowest portion of the end of the rail can cause a risk of neck entrapment. The FDA recommends this space to be less than 2 3/8 inches.

5) Between split bed rails, or between a head and foot set of rails on the same side. When partial length side rails are used on the same side of the bed, the space between them may present a risk of either chest or neck entrapment.

6) Between the end of the rail and the side edge of the head or foot board. A gap between the side edge of the headboard or footboard and the end of the bed rail can present the risk of body entrapment.

7) Between the end of the mattress and the head or foot board. The risk of head entrapment increases when there is too large of a space between the end of the mattress and the inside surface of the headboard or footboard.

How do I Choose a Safety Bed?

When choosing a safety bed, it needs to be comfortable besides being a safe environment to sleep in. It should also meet the FDA guidelines for falls and entrapment. There are five basic needs to consider; which one will suit the needs of the individual, what features or functions are needed in the foundation, what size of bed is most appropriate, what wood finish would look best in the room, and whether padding is needed.

1) To determine which safety bed fits the needs of the individual, first decide the height of safety rail protection that is desired above the mattress. This choice should be made based on the user’s activity level. Questions to answer are: how active is the user, how much safety rail above the mattress is needed, can they sit up, and can they pull to kneel or stand in the bed. Keeping the answers to these questions in mind will help to determine which safety bed is most appropriate. Also, consider the bed’s transfer height, and whether the user will be going from the bed to a wheelchair independently or if a caregiver will need the bed higher for lifting transfers.

2) Determine what features or functions are needed in a foundation. There are four different types of foundations: fixed, manual, articulated and hi-lo.

A fixed foundation offers a fixed height and no articulation. The mattress will lie flat on a box spring or board, and depending on the model chosen there will be more than one choice for positioning in that fixed height when being installed. The positioning choice that is made when installed will determine the height of safety rail protection above the mattress. For example, if choosing to install at the lower position, this will increase the protection height above the mattress. The height of the fixed foundation can be changed at any time.

A manual foundation can offer different choices, using hand cranks with a hydraulic system. They are normally easy to use with little effort by the operating caregiver. When not in use, the hand crank should fold up into the handle’s shaft. The different choices are: 1) manual hi-lo without articulating feature, this raises and lowers the mattress height with one crank, 2) manual articulating without the hi-lo feature, this raises and lowers the head and/or knee section of the bed with a combination of two cranks, 3) electric articulating with manual hi-lo feature, this allows the user to raise and lower the head and/or knee section of the bed electronically with a remote, and raise or lower the mattress with a manual crank, and 4) manual hi-lo with articulating feature, this raises and lowers the mattress height and raises and lowers the head and knee section with a combination of hand cranks. The hand cranks are easy to use with minimal effort by the caregiver with a hydraulic system.

An articulated foundation offers a fixed height with an electric frame. With a remote control the head and/or knee section can be elevated with the push of a button. Like the fixed foundation, there are different height positions to choose from when being installed, and the height choice that is made will determine the height of safety rail protection.

A hi-lo foundation offers full articulation with a hi-lo electric frame. Besides elevating the head and knee section, the mattress surface can be raised or lowered by remote control. This type of foundation is considered a fully electric bed. Depending on where the mattress is positioned within the frame will determine the amount of safety rail protection above the mattress.

3) Choosing the proper bed size is important for comfort and safety. Most safety beds are available in twin, full or queen sizes.

4) There are different types of wood finishes to select, dependent upon the room design. Maple is the lightest of the woods and shows very little wood grain, oak is a bit darker but still a light wood with a little more wood grain, cherry is darker than the previous woods and will darken naturally over time, and mahogany is the darkest of the woods and will darken over time with good wood grain. Some safety beds come in colors that can be mixed and matched for personal preference.

5) A safety bed with or without padding depends on specific needs. It is important to decide this when purchasing the bed in order to save extra expense if this option is decided on later. Padding normally covers all four sides of the bed, not just one or two sides. It is highly recommended to never remove any of the pads because this will cause a gap between the mattress and the sides of the bed, potentially creating a risk for entrapment.
